How care information supports navigation, who may receive updates, and how to make a request.

Partner programs and direct services
MySeema ("MySeema," "we," "us") uses care information to provide the coordination and navigation arranged with you or your healthcare organization. This page supplements our Privacy Policy and describes consumer health information in direct services where applicable.
When MySeema handles protected health information as a business associate for a healthcare organization, the applicable business associate agreement and that organization’s privacy notice govern the arrangement. Contact your provider for its HIPAA Notice of Privacy Practices; MySeema can help route that request. This explanatory page is not a replacement for a covered healthcare provider’s required notice.
For direct services, the relevant privacy protections depend on the information, the parties, and applicable law. Private payment alone does not determine HIPAA status. We do not ask you to waive applicable health-information protections as a condition of navigation.
Care information and its sources
Depending on the support requested and permissions provided, care information may include:
- Names, contact details, date of birth, authorized representatives, and the fact that a person seeks or receives a health-related service.
- Provider names, appointment dates, referral details, discharge or visit instructions, care plans, visit summaries, and related records.
- Conditions, symptoms, medication and refill information, laboratory or monitoring information, therapy and recovery needs, and questions for the care team.
- Insurance and billing information relevant to coordination, language and communication preferences, and practical barriers such as travel or access needs.
Sources include the person receiving support, authorized family or representatives, treating providers and their records, and other service organizations involved in the requested coordination. Information may reveal a health need even when it does not contain a diagnosis. Please use the care-information channel arranged with the team.
Why we use and share it
We use the information needed to arrange support, prepare for appointments, communicate with providers, document what happened, pursue next steps, maintain service records, and meet applicable legal requirements. We obtain authorization or consent where required. Agreeing to navigation does not give every family member or organization access to your information.
The information shared depends on the task: appointment and referral details for scheduling; relevant medication information for a refill request; records or questions for a provider; and an authorized summary for a family contact. Recipient categories may include treating providers and their offices, pharmacies, insurers or billing offices, authorized family or representatives, and service providers supporting communications, records, and service administration. Appropriate contractual and legal requirements apply.
We do not authorize public posting or advertising use of a person’s care record merely because they use MySeema. A testimonial, photograph, or public story requires separate appropriate permission. Uses or disclosures requiring separate consent or authorization must be explained before they occur.
Who controls family updates
The person receiving support, or a representative with legal authority, controls which family members or others may receive updates and what may be shared, subject to applicable law. Paying for a membership or gifting a visit does not by itself create authority to make healthcare decisions or receive medical information.
You may change authorized contacts or withdraw an authorization prospectively by contacting us. Withdrawal does not reverse a disclosure already made in reliance on a valid permission or prevent a use required or otherwise permitted by law. We may verify identity and representative authority before acting.
Access, correction, deletion, and withdrawal
Contact support@myseema.com to ask what information we hold, request a copy or correction, request deletion where available, withdraw consent where applicable, or ask us to restrict a communication channel. Tell us the type of request and how to reach you; we will arrange appropriate identity verification. You may use an authorized representative where permitted.
For provider-controlled records, the provider may need to make the decision; MySeema assists as required by its agreement. Applicable retention, legal, and record-integrity obligations can limit deletion or changes to an original record. We explain applicable limits and respond within the deadlines required by law.
Where Washington’s My Health My Data Act applies to direct-service information, you may request confirmation and access, information about applicable sharing, withdrawal of consent, and deletion as provided by that law. You may appeal a refusal by replying to our response or emailing support@myseema.com. We explain the outcome and applicable complaint options. These rights are subject to the law’s scope and exemptions; this notice does not reduce them.
Concerns, incidents, and privacy notices
Report a concern to support@myseema.com and identify it as a privacy request. We do not retaliate for requesting privacy rights or raising a concern. You may also contact the relevant healthcare organization, your state attorney general, or the U.S. Department of Health and Human Services Office for Civil Rights where HIPAA applies.
If a breach requires notice, we provide or coordinate notice under the applicable law and service agreements. A member’s direct-service records and records handled for a provider may follow different notification processes.
